The Local Choice Benefits (Health, Dental, Vision) - Please reach out to your Benefits Department within 30 days of your date of hire.
Legal & ID Theft Benefits - Please contact the provider within 30 days of your hire date. Additional enrollment instructions and contact information can be found within the benefits guide.

– Anthem Blue View Vision
– Employee Maximum $4,400/year
– Family Maximum $8,750/year
HSA plans can only be established in conjunction with a qualified High-Deductible Health Plan (HDHP)
Flexible Spending Accounts: Ameriflex– Medical Reimbursement Maximum: $3,400/year
– Dependent Care Reimbursement Maximum: $7,500/year | $100/year Min
You will need to re-enroll in the Flexible Spending Accounts if you want them to continue next year.
