Existing Supplemental/Voluntary Policies
Your individual supplemental/voluntary policies through Colonial Life are portable! To transfer your benefits from payroll deduction to direct billing, please call PGB within 30 days of becoming unemployed, switching careers, or retiring. You will need to fill out and provide the below form.

Frequently Asked Questions
What are the eligibility requirements?
As long as you’re not employed by a current PGB account, you’re qualified and there are no eligibility requirements! From the retired to the unemployed, anyone can sign up, at anytime no matter your current status.
If you are transferring from one active PGB account to another, please DO NOT sign up for these individual policies. Please contact our Service Center so that we may help you with transferring your benefits.
What kind of plans are offered?
For both Dental and Vision, there are multiple plans for you to choose from to suit your needs. There are also tiered rates that include Individual, Individual +1, and Family.
For Telemedicine, there are two plan options: Individual and Family.
How soon can I get coverage?
As soon as the next day!*
*For Dental applications placed prior to the 20th of the month, you may choose to start your coverage the 1st of the current month, or the 1st of the next month. For Vision, your coverage will begin the next business day.
Can I get coverage for my family?
Yes! Individual and Family coverage options are available for Dental, Vision, and Telemedicine.
How do I pay for individual plans?
At the time of sign up, you'll have the choice to connect a bank account or debit/credit card as your form of payment.
