YOU CAN MAKE THE FOLLOWING BENEFIT ELECTIONS ONLINE DURING THE ENROLLMENT PERIOD:
– Enroll, change or cancel The Local Choice benefits (Anthem Health, Delta Dental & Blue View Vision).
– Enroll, change or cancel your Health Savings Account (HSA).
– Enroll, change or cancel your Vision Insurance (EyeMed stand-alone policy).
– Enroll, change or cancel your Dental Insurance (Delta stand-alone policy).
– Enroll, change or cancel your Colonial products.
PLEASE NOTE: If you are making changes to The Local Choice election, do not forget to print out the application, sign, and return it to the Benefits Counselor assigned to your location, or Donna Henderson at the Main Office.
