Important Information About Your Enrollment

During your open enrollment period, a PGB Benefits Representative will be available by appointment to meet with you one-on-one to help you evaluate your benefits based on your individual and family needs, answer any questions you may have, and assist you in the enrollment process.
BENEFIT ELECTION OPTIONS
– Enroll in, change or cancel The Local Choice benefits (Health, Dental, and Vision).*
– Enroll/Re-Enroll in Flexible Spending Accounts.**
– Enroll in, change or cancel Colonial coverage.
*All employees are required to meet with a Benefit Counselor to complete a The Local Choice enrollment form, as well as sign or decline for Colonial Insurance Products and Flexible Spending Accounts.
**You will need to re-enroll in the Flexible Spending Accounts if you want them to continue each year.
