COMPLETE THE STEPS BELOW TO BEGIN THE ENROLLMENT PROCESS:
1. Go to https://harmonyenroll.coloniallife.com
– Enter your User Name: CIT2c9w- and then Last Name and then Last 4 of Social Security Number (CIT2c9w-SMITH6789)
– Enter your Password: Four digit Year of Birth and then Last 4 of Social Security Number (19766789)
2. The screen prompts you to create a NEW password [__________].
3. Choose a security question and enter answer [__________].
4. Click on ‘I Agree’ and then ‘Enter My Enrollment’.
5. The screen shows ‘Me & My Family’. Verify that the information is correct and enter the additional required information (title, marital status, work phone, e-mail address). Click ‘Save & Continue’ twice.
6. The screen allows you to add family members. It is only necessary to enter family member information if adding or including family members in your coverage. Click ‘Continue’.
7. The screen shows updated personal information. Verify that the information is correct and make changes if necessary. Click ‘Continue’.
8. The screen shows ‘My Benefits’. Review your current benefits and make changes/selections for the upcoming plan year.

You may enroll online in Long-Term Disability coverage.
– CANCER ASSIST
You may enroll online in Cancer Assist coverage.
– SHORT-TERM DISABILITY
You may enroll online in Short-Term Disability coverage.
– ACCIDENT 1.0
You may enroll online in Accident 1.0; however, persons over age 64 applying for coverage and employees wishing to purchase an individual policy for their spouse should speak with the Benefits Representative.
– GUNSHOT WOUND POLICY
You will need to speak with the Benefits Representative in order to enroll in the Gunshot Wound Policy.
– MEDICAL BRIDGE
You may enroll online in Medical Bridge coverage.
– CRITICAL ILLNESS 6000
You may enroll online in Critical Illness 6000 coverage.
– TERM LIFE 5000
You may enroll online in Term Life 5000; however, employees wishing to purchase an individual policy for their spouse should speak with the Benefits Representative.
– WHOLE LIFE 5000
You may enroll online in Whole Life 5000; however, employees wishing to purchase an individual policy for their spouse should speak with the Benefits Representative.
9. Click ‘Finish’.
10. Click ‘I Agree’ to electronically sign the authorization for your benefit elections.
11. Click ‘Print a copy of your Elections’ to print a copy of your elections, or download and save the document. Please do not forget this important step!
12. Click ‘Log out & close your browser window’ and click ‘Log Out’.

QUALIFICATIONS:
– You must be a full-time employee working 30 hours or more per week.
IMPORTANT FACTS:
– The plan year for Colonial Insurance products and Reliance Standard Long-Term Disability lasts from July 1, 2021 through June 30, 2022.
– Deductions for Colonial Insurance products and Reliance Standard Long-Term Disability will begin July 2021. The Nationwide Pet Insurance plan is available by Direct Billing only. No deductions will be taken via payroll deduction.
– If signing up for any coverage on your spouse and/or children, please have their dates of birth and social security numbers available when meeting with the Benefits Representative.
– Elections made during this enrollment period CANNOT BE CHANGED AFTER THE ENROLLMENT PERIOD unless there is a family status change as defined by the Internal Revenue Code. Examples of a family status change are: marriage, divorce, death of a spouse or child, birth or adoption of a child, termination or commencement of a spouse’s employment, or the transition of spouse’s employment from full-time to part-time, or vice-versa.
– Once a family status change has occurred, an employee has days to notify the North Carolina Service Center at 1-888-662-7500 to request a change in elections.
– The Colonial Cancer plan and the Health Screening Rider on the Colonial Accident and Colonial Medical Bridge plan have a 30-day waiting period for new enrollees. Coverage, therefore, will not begin until July 31, 2021.
• Additionally, some policies may include a pre-existing condition clause. Please read your policy carefully for full details.
• Please be aware there are certain coverages that may be subject to federal and state tax when premium is paid by pretax deduction or employee contribution.

Group Short Term Disability (STD) program provided for its participant by the Sponsor and administered by Anthem Life Insurance Company helps provide financial protection for covered members by promising to pay a weekly benefit in the event of a covered disability.

How does a healthcare FSA work?
A healthcare FSA is a flexible spending account that allows you to set aside pre-tax dollars for eligible medical, dental, and vision expenses for you and your dependents, even if they are not covered under your primary health plan.
You choose an annual election amount. At the beginning of the plan year, your account is pre-funded and your full contribution is immediately available for use. Your election amount is then deducted from your paychecks in equal installments throughout the year.
Why should I enroll in a healthcare FSA?
Almost everyone has some level of predictable and non-reimbursable medical needs.
If you expect to incur medical expenses that won’t be reimbursed by another plan, you’ll want to take advantage of the savings this plan offers. Money contributed to a healthcare FSA is free from federal and most state taxes and remains tax-free when it is spent on eligible expenses. Helpful hints
– Your full election amount is available on the first day of the plan year, which means you’ll have access to the money you need, when you need it.
– You can’t change your election amount during the plan year, unless you experience a change in status or qualifying event.
– Save your receipts when you spend your healthcare FSA dollars. You may need itemized invoices to verify the eligibility of expenses or for reimbursement requests.
– If your employment terminates before the end of the plan year, your account will terminate unless you are eligible for, and elect, COBRA coverage.
– Any unused funds that remain in your account at the end of the plan year will be forfeited. Plan carefully and use all the money in your healthcare FSA by the end of the plan year.
Dependent Care FSA

Participating in a dependent care flexible spending account (DCA) is like receiving a 30% discount from your care provider.
How does a dependent care FSA work?
A dependent care FSA is a flexible spending account that allows you to set aside pre-tax dollars for dependent care expenses, such as daycare, that allow you to work or look for work.
You choose an annual election amount, up to $5,000 per family. The money is placed in your account via payroll deduction, in equal installments, and then used to pay for eligible dependent care expenses incurred during the plan year.
Why should I enroll in a dependent care FSA?
Child and dependent care is a large expense for many families. Millions of people rely on child care to be able to work, while others are responsible for older parents or disabled family members.
If you pay for care of dependents in order to work, you’ll want to take advantage of the savings this plan offers. QUALIFICATIONS:
• Employees must work a minimum of 17.5 hours per week to participate in benefits.
IMPORTANT FACTS:
• The plan year for Aetna Health, Delta Dental, EyeMed Vision, Colonial Insurance products and Spending Accounts lasts from July 1, 2021 through June 30, 2022.
• Deductions for Aetna Health, Delta Dental, EyeMed Vision, Colonial Insurance products and Spending Accounts will begin July 2021 for 12-month employees and September 2021 for 10-month employees.
• Health FSA Rollover Provision: Your employer provides the rollover option for your FSA plan. Please see the Flexible Spending Account section of your benefit booklet for more information on this provision.
• If signing up for any coverage on your spouse and/or children, please have their dates of birth and social security numbers available when meeting with the Benefits Representative.
• If you will be receiving a new debit card, please be aware that it may take up to 30 days following your plan effective date for your card to arrive. Your card will be delivered by mail in a plain white envelope. During this time you may use manual claim forms for eligible expenses. Please note that your debit card is good through the expiration date printed on the card.
• Elections made during this enrollment period CANNOT BE CHANGED AFTER THE ENROLLMENT PERIOD unless there is a family status change as defined by the Internal Revenue Code. Examples of a family status change are: marriage, divorce, death of a spouse or child, birth or adoption of a child, termination or commencement of a spouse’s employment, or the transition of spouse’s employment from full-time to part-time, or vice-versa.
• Once a family status change has occurred, an employee has 30 days to notify the Pierce Group Benefits Service Center at 1-800-387-5955 to request a change in elections.
• Flexible Spending Account expenses must be incurred during the Plan Year in order to be eligible for reimbursement.
• An employee has 90 days after the plan year ends to submit claims for spending account expenses that were incurred during the plan year. Please note that if employment terminates during the plan year, that employee’s plan year ends the day employment ends. The employee has 90 days after the termination date to submit claims.
• With Dependent Care Flexible Spending Accounts, the maximum reimbursement you can request is equal to the current account balance in your Dependent Care account. You cannot be reimbursed more than has actually been deducted from your pay.
• As a married couple, one spouse cannot be enrolled in an FSA at the same time the other is contributing to an HSA.
• The Health Screening Rider on the Colonial Medical Bridge plan have a 30-day waiting period for new enrollees. Coverage, therefore, will not begin until July 31, 2021.
• Additionally, some policies may include a pre-existing condition clause. Please read your policy carefully for full details.
• Please be aware there are certain coverages that may be subject to federal and state tax when premium is paid by pretax deduction or employee contribution.
• An employee taking a leave of absence, other than under the Family & Medical Leave Act, may not be eligible to re-enter the Flexible Benefits Program until the next plan year. Please contact your Benefit Administrator for more information.
