QUALIFICATIONS:

  • Employees must work 20 hours or more per week with a contract.

 

IMPORTANT FACTS:

  • The plan year for Colonial Insurance products, Spending Accounts and Superior Vision lasts from October 1, 2020 through September 30, 2021. The Local Choice Benefits are included in the booklet for informational purposes only.
  • Health FSA Rollover Provision: Your employer provides the rollover option for your – FSA plan. Plan participants should note that up to $500 of any unused funds from the current plan year will be rolled over into your FSA balance for the upcoming plan year. Please see the Flexible Spending Account section of your benefit booklet for more information on this provision.

 

  • Deductions for Colonial Insurance products, Spending Accounts and Superior Vision will begin October 2020.
  • The 2019-2020 Flexible Spending Account Plan includes a grace period from October 1, 2020 through December 15, 2020. Therefore, you have from July 1, 2019 through December 15, 2020 to incur qualified expenses eligible for reimbursement in the Medical and Dependent Care Spending Accounts. If you do not incur qualified expenses eligible for reimbursement by December 15, 2020 and/or file for reimbursement by December 31, 2020 any contributions are forfeited under the use-or-lose it rule.

 

  • If signing up for any coverage on your spouse and/or children, please have their dates of birth and social security numbers available when meeting with the Benefits Representative.

 

  • If you will be receiving a new debit card, whether you are a new participant or to replace your expired card, please be aware that it may take up to 30 days following your plan effective date for your card to arrive. Your card will be delivered by mail in a plain white envelope. During this time you may use manual claim forms for eligible expenses.  Please note that your debit card is good through the expiration date printed on the card.

 

  • Elections made during this enrollment period CANNOT BE CHANGED AFTER THE ENROLLMENT PERIOD unless there is a family status change as defined by the Internal Revenue Code. Examples of a family status change are: marriage, divorce, death of a spouse or child, birth or adoption of a child, termination or commencement of a spouse’s employment, or the transition of spouse’s employment from full-time to part-time, or vice-versa.

 

  • Once a family status change has occurred, an employee has 30 days to notify the Pierce Group Benefits Service Center at 1-800-387-5955 to request a change in elections.
  • Flexible Spending Account expenses must be incurred during the Plan Year in order to be eligible for reimbursement.

 

  • An employee has 90 days after the plan year ends to submit claims for medical reimbursement and/or dependent care expenses that were incurred during the plan year. Please note that if employment terminates during the plan year, that employee’s plan year ends the day employment ends. The employee has 90 days after the termination date to submit claims.

 

  • With Dependent Care Flexible Spending Accounts, the maximum reimbursement you can get is equal to the current account balance in your Dependent Care account. You cannot be reimbursed more than has actually been deducted from your pay.

 

  • The Health Screening Rider on the Colonial Medical Bridge plan have a 30-day waiting period for new enrollees. Coverage, therefore, will not begin until October 31, 2020.

 

  • Additionally, some policies may include a pre-existing condition clause. Please read your policy carefully for full details.

 

  • Please be aware there are certain coverages that may be subject to federal and state tax when premium is paid by pretax deduction or employee contribution.

 

  • An employee taking a leave of absence, other than under the Family & Medical Leave Act, may not be eligible to re-enter the Flexible Benefits Program until the next plan year. Please contact your Benefit Administrator for more information.

IMPORTANT NOTICE REGARDING DENTAL PLAN WAITING PERIODS:

Enrolling In This Dental Benefit Plan

You are a timely enrollee if you apply for coverage and/or add dependents within a 30-day period of when you first become eligible for coverage under this dental benefit plan or within 30 days following a qualifying event as described below. Once dental coverage has terminated, regardless of the reason, you may not re-enroll, unless a qualifying event occurs.

If you apply for coverage at a time, which does not qualify you or your dependents as timely enrollees as stated above, then you are considered late enrollees. Late enrollees have no waiting period for diagnostic and preventive services. For all other dental services, where timely enrollees have no waiting period, late enrollees have a 12-month waiting period. Where timely enrollees have waiting periods, these waiting periods are doubled for late enrollees. See “Waiting Periods.”

Waiting Periods

There is no waiting period for members to receive benefits for diagnostic and preventive, or basic services, except for late enrollees. However, there is a 12-month waiting period for major services and a 12-month waiting period for orthodontic services. These waiting periods are doubled for late enrollees. Eligible children who are added as a result of a court order are not subject to a waiting period. Waiting periods are waived for timely enrollees who can show proof of prior dental coverage, with a maximum gap in coverage of no more than 63 days. If a waiting period applies, see the Dental Coverage chart in “Covered Services.”
